When We Turn Down All-on-4 Candidates

Most dental websites say you might be a candidate for All-on-4 implants. Then they invite you to call and find out. We do things a little differently. We’d rather tell you upfront when All-on-4 isn’t the right fit, and why.

That might sound strange for a practice that offers the procedure, but recommending the wrong treatment doesn’t help anyone. Not you, not us. It leads to failed implants, wasted time, and a patient who trusted us and got burned.

So let’s talk honestly about when we turn down All-on-4 candidates at our Virginia Beach office, and what we offer instead. If you’ve been told you need dentures or you’re worried you won’t qualify for implants, this is for you.

Why We Evaluate Candidacy So Carefully

All-on-4 works by placing four implants at strategic angles to support a full arch of teeth. It’s an excellent solution for many patients, but it isn’t magic. It still depends on your body’s ability to heal and hold onto those implants.

Dr. Cappy Sinclair has advanced training in cosmetic and functional dentistry through the American Academy of Cosmetic Dentistry. That background means he doesn’t just look at whether implants can be placed. He considers how your whole mouth will function and look over the long term. Our team also includes an oral and maxillofacial surgeon who weighs in on complex cases.

Between the two, we can usually tell you honestly whether All-on-4 is your best path. Sometimes the answer is yes, or not yet. Sometimes it’s a different treatment altogether.

Reason 1: You Don’t Have Enough Bone, Even for Angled Implants

All-on-4 is designed to work around bone loss. The angled placement lets us use the denser bone in the front of your jaw and avoid areas where bone has shrunk. That’s why many patients who were told they’d need dentures end up qualifying.

But there’s a limit. If bone loss is severe enough, even angled implants won’t have enough to hold onto.

What we offer instead: For some patients, a bone graft can rebuild enough structure to make implants possible later. Our oral and maxillofacial surgeon handles grafting in-house, so you won’t be bounced to a different office. For others, a hybrid approach or an implant-supported denture might be the better fit.

The important part is that not qualifying today doesn’t mean you never will. It means we need a different starting point.

Reason 2: You Have Active Gum Disease or Infection

Implants need a healthy foundation. If your gums are actively infected, placing implants into that environment is asking for trouble. The infection can spread to the implant site and cause the implant to fail.

We won’t place implants on top of an active infection. That’s not being cautious. That’s being responsible.

What we offer instead: We treat the gum disease first. That might mean deep cleanings, laser therapy, or other periodontal treatment. Once your gums are healthy, we revisit the implant conversation. Many patients who start here end up with successful All-on-4 results. They just needed to fix the foundation first.

Reason 3: A Medical Condition Affects Your Healing

Certain health conditions and medications can slow healing or interfere with how your body accepts implants. Uncontrolled diabetes is a common one. So are some autoimmune conditions and medications that suppress the immune system. Smoking is another big factor.

We’re not here to judge. We’re here to be honest about risks.

What we offer instead: Depending on your situation, we may ask you to work with your doctor to better control your condition before surgery. If you smoke, we’ll talk about what quitting for a period before and after surgery can do for your success rate. In some cases, we proceed with extra precautions. In others, we recommend a different tooth replacement option.

Either way, you’ll leave your consultation knowing exactly where you stand and why.

Reason 4: You’re Not Ready, and That’s Okay

Sometimes the issue isn’t medical. It’s timing. Maybe you need to handle another health issue first. Maybe you’re not sure All-on-4 is what you want. What We Offer When All-on-4 Isn’t the Answer

If All-on-4 isn’t right for you, that doesn’t mean you’re out of options. Depending on your situation, we might recommend:

Bone grafting followed by implants later. Rebuild the foundation first.
An implant-supported denture. Fewer implants, still stable, still removable if you prefer.
Traditional dentures. Sometimes the simplest answer is the right one.
A partial denture or bridge. For patients missing fewer teeth.
Treatment for gum disease first. Then revisit implants.
