Single vs. Multiple vs. All-on-4

One of the first questions patients ask us at our Virginia Beach office is how many implants they actually need. Implant treatment isn’t one-size-fits-all. Someone missing a single tooth has very different needs than someone missing a full arch. And choosing the wrong solution can mean wasted money, unnecessary procedures, or results that don’t hold up.

Here’s how to think through the options so you can walk into a consultation knowing what to expect.

Start With Your Situation, Not the Treatment

Most people approach this backward. They hear about a treatment, like All-on-4, and wonder if it applies to them. A better approach is to start with what you’re actually missing and work forward.

Ask yourself three questions:

How many teeth am I missing?
Are the missing teeth in a row, or scattered?
How healthy are the teeth I still have?

Your answers point toward one option more than the others. Let’s walk through each scenario.

If You’re Missing One Tooth: Single Implant

A single missing tooth is easy to ignore, especially if it’s toward the back of your mouth. But ignoring it causes problems. The teeth on either side can drift into the gap. Your bite can shift. And the bone under the missing tooth starts to shrink because no root stimulates it.

For one missing tooth, a single dental implant is usually the best answer. A single implant replaces the tooth root and the crown. It stands on its own. That means we don’t have to touch the healthy teeth next to it. Compare that to a traditional dental bridge, which requires grinding down the two neighboring teeth to support the replacement. You’re essentially damaging two healthy teeth to fix one gap.

A single implant also prevents bone loss in that spot, because the implant stimulates the jaw the way a natural root would.

Best for: One missing tooth, healthy neighbors, enough bone in that spot.

If You’re Missing Two or Three in a Row: Implant-Supported Bridge

When you’re missing a few teeth in a row, you don’t necessarily need one implant per tooth. Two implants can support a three-tooth bridge. That’s the most efficient and cost-effective approach.

We place two implants, one at each end of the gap. Once they heal, we attach a custom bridge that fills the space. The bridge is fixed, so it doesn’t come out. It looks and functions like natural teeth.

This approach also protects the teeth on either side of the gap, because we don’t have to grind them down the way a traditional bridge requires.

Best for: Two to three missing teeth in a row, healthy bone at the ends of the gap.

If You’re Missing Several Teeth, Scattered: Multiple Implants

Sometimes teeth are missing in different spots, not in a neat row. In those cases, we may place several individual implants, each supporting its own crown. Or we might combine implants with other restorations, depending on what’s healthy and what isn’t.

This is where a detailed exam really matters. Two patients can look similar on the surface and need very different plans. That’s why we take 3D imaging and evaluate your whole mouth before recommending anything.

Dr. Sinclair has advanced training through the American Academy of Cosmetic Dentistry, so he considers both function and appearance. He’s planning how your bite will work and how your smile will look long-term.

Best for: Several missing teeth in different areas, with healthy bone in the right spots.

If You’re Missing a Full Arch: All-on-4

When an entire row of teeth is missing or failing, All-on-4 is the strongest option. Instead of one implant per tooth, four implants support a full arch. Two go in the front, two go in the back at an angle.

That angled placement is what makes All-on-4 so effective. It lets us use the denser bone in the front of your jaw and avoid areas where bone has shrunk. Many patients who were told they’d need full dentures qualify for All-on-4 instead.

You leave with a temporary set of teeth the same day as surgery. Once the implants heal, we attach your final, custom restoration.

Best for: A full arch of missing or failing teeth, even with some bone loss.

What If You Have Some Healthy Teeth Left?

This is where things get more nuanced, and where many patients get confused.

If you still have some healthy teeth, you might not need to replace the whole arch. We might be able to place implants only where teeth are missing and keep the healthy ones. That’s the better path because it preserves your natural teeth.

But sometimes the remaining teeth are compromised. They may be cracked, decayed, or loose. In those cases, keeping them can cause more problems down the road. We’ll walk you through the trade-offs honestly.

The key point: you don’t have to lose all your teeth to benefit from implant treatment. And you don’t have to keep teeth that are causing trouble.

Why the Recommendation Shouldn’t Depend on What the Practice Offers

Here’s something worth paying attention to. Some practices offer only one or two implant solutions, so their recommendations tend to point in that direction. A practice that only does All-on-4 will find that All-on-4 is the answer. A practice that only does single implants will steer you there.
